Living Well With a Chronic Condition (LWWCC)
What is it?
LWWCC is a group education program for those with one or more chronic conditions such as diabetes, heart disease, COPD or chronic kidney disease. It will allow participants to interact with and learn from your peers in a supportive, friendly environment while providing practical information and skills to better manage your chronic condition, such as fatigue management, mindfulness, appropriate medication use, how to evaluate new treatments and much more.
Who runs the program?
LWWCC is run by two trained Stanford chronic condition self management facilitators.
How often does it run?
The workshops are held regularly throughout the year and involve a 2-hour session every week for six weeks.

LWWCC is held in Rockingham, Mandurah, Pinjarra and Guildford.
Accessing the program
LWWCC can be accessed by GP referral or by self-referral. Referral forms can be found under the "For GPs" tab on this website. Call us on 1300 706 922 for more information and schedules.
The cost of attending LWWCC is $30 for the full 6 week program.
