ICDC is our Kalgoorlie-based chronic health care program, funded by WAPHA.
The ICDC program seeks to improve self-management for those individuals living with a chronic condition by providing access to allied health professionals in the central and northern Goldfields, and to help them access any other services they might need.
The Care Coordinator will see you initially to gather your medical history, and any other information that is relevant to your health and wellbeing. They will also discuss your health goals set by your doctor and yourself. This goal-setting is a crucial part of the ICDC program, ensuring that our healthcare matches our clients’ personal wants and needs.
What supports can you expect from our ICDC program?
Care coordination – We help formulate your healthcare plan and support you to set personal health goals.
Dietetics – Assess your diet against any health needs you have to ensure you maximise your health and wellbeing, and help you develop good food habits to better manage your conditions.
Diabetes education – Review of blood results, providing information about your diabetes, and help to formulate a plan for your diabetes management.
Exercise physiology – Evaluate your body’s movement and functional capacity, and design an individualised exercise program to help manage your health condition, increase mobility, strength, flexibility and balance to help you reach your goals.
Podiatry – Assess your feet and help you stay on them for as long as possible.
